VACCINATION STATUS FORM - DUE BY 8/1
Santa Clara County is requiring businesses, including nonprofits, to determine the vaccination statuses of their employees and volunteers. In kind, Cambrian is requiring all players participating in our September set to complete and return the below form as soon as possible. We will not disclose your personal vaccination status to anyone, but if asked we will share what percentage of the orchestra is vaccinated, as some players may want to know this information before they agree to participate. Please send completed forms to [email protected] as soon as possible, and no later than August 1.

COVID WAIVERS - DUE BY 8/15 (UPDATED 7/29/21)
All musicians joining us for our September set are required to sign and return a Cambrian COVID Waiver by 8/15. This agreement states the following:

Your attendance at any Cambrian event is your attestation that you have not had COVID-19 symptoms within the past 14 days, nor have you had close unprotected contact (within 6 feet for 15 minutes or longer, without face covering) with any confirmed or suspected cases of COVID-19 within the past 14 days.
You agree to notify Cambrian if you develop COVID-19 symptoms within 14 days of attending any Cambrian event.
You will not sue Cambrian if you develop COVID-19.

Please return signed forms to [email protected] by 8/15. All waivers must include a signature - you may like to check out Adobe Fill & Sign or DocuSign to sign electronically. If we do not receive a signed COVID waiver from you by 8/15, we will ask you not to play this set - we will be happy to have you rejoin at a later date. If returning electronically poses difficulty for you, please let us know and we will be happy to work with you.

MASKS AND SOCIAL DISTANCING (UPDATED 8/8/21)
To comply with the new county mask mandate, our board asks all players to adhere to the following:

Anyone who is unvaccinated, regardless of instrument, must wear a mask at all times within our rehearsal and performance venues.
All strings and percussion must wear a mask at all times in our rehearsal and performance venues, even if you are vaccinated.
Winds/brass must wear a mask when not playing within our rehearsal and performance venues. You may remove your mask when playing if you are vaccinated. You may be interested to check out these musician's masks/PPE although we will not require them if you are vaccinated.
We ask players to practice social distancing as much as possible while at Cambrian events.

Please consider these rules to be in effect until further notice. If you do not feel comfortable with these requirements, we ask you not to play this set - we will be happy to have you rejoin at a later date. Please note that Hammer Theatre is currently requiring all audience members to wear masks, regardless of vaccination status.

UPDATED 4/25/22 - NEW COVID POLICIES - SUBJECT TO CHANGE
We have modified our COVID policies in light of the recent changes at the state and county levels. These policies are subject to change.

All players must be fully vaccinated for COVID-19. Boosters are not required.
Send proof of vaccination and a signed Cambrian COVID Waiver to [email protected] by 5/20/22 if you have not already provided this season.
Masks are strongly recommended but are not required for any musician.
As of 4/25/22, we intend to follow Hammer Theatre's current policies for audience attendance: Masks are recommended for all patrons. Proof of vaccination is not required. See https://hammertheatre.com/faq/.
